What is the Diabetes Multi-disciplinary team?
The purpose of the Northamptonshire Diabetes Multi – disciplinary Team (NMDT) is to promote care closer to home, facilitating a seamless service between acute and primary care and avoiding acute hospital admissions and accelerating safe discharge. The delivery of high quality care, efficiently and cost effectively, in accordance with the Northamptonshire Diabetes Care Pathway, is a priority. Our focus is to enable people with diabetes to remain healthy by working collaboratively to promote self-management and avoid acute and long-term complications of diabetes. We promote this through 1-1 and group education contacts with individuals who have diabetes and by supporting, educating and increasing the skills of other healthcare professionals who provide care to people with diabetes.

“Me and My Diabetes” a programme for people with a learning disability and their carers

The Diabetes MDT consists of the following health professionals:

Diabetes Specialist Nurses
Diabetes Specialist Dieticians
Diabetes Specialist Podiatrists
Diabetes Specialist Support Workers
Administration team
