Service description:
For clients with mental health needs:
Midland Heart provides a wide range of services to help those managing mental heath issues. This includes customers who only require a little bit of support to maintain their independence through to customers that have much more complex issues.

We are committed to improving the quality of life for customers with mental health issues. We work with customers, their families, carers, advocates and other organisations to support our customers to live the life they choose and we do this with dignity and respect.

We can help customers with paying bills, basic living skills, personal care and managing household chores. We can also support customers with medication management. By working in partnership with customers we can support them to develop and maintain relationships, access to employment and training, and access social activities.

For clients with LD or PD:
Midland Heart delivers services to adults with a physical or learning disability, including people with dual diagnosis, sensory impairment, complex needs, acquired brain injury, behaviour that challenges and autism (ASD).

All our services and support are outcome focused and recognise the customer as the expert of themselves. We deliver tailored support and care packages and are keen to explore innovative ways of maximising opportunities, life experiences and choices, for example through different support models and the use of assistive technology. We work in partnership with customers, their families, carers, advocates and other organisations to support people to live the life they choose.

Midland Heart offers a range of accommodation options and support packages; residential care, shared supported living, reablement services, transitional services and independent living (support in your own home).
